Talk to People Who Can Help

When you are thinking about moving out of your facility, call the Senior LinkAge Line® (SLL) at 1-800-333-2433 or Chat with a Hub expert. Trained staff can talk to you about all your options and help you figure out what option is best for you.

You can also get advice about living in the community by talking to other people with disabilities similar to your own — a good place to find someone is at your local independent living center, Self-Advocates Minnesota, and the Brain Injury Alliance.

Resources for Planning Your Transition

The Center on Medicare and Medicaid Services (CMS) has a Consumer Discharge Checklist that gives you a good idea about the things you need to think about before you leave a facility, and how to plan for getting your care at home.

Access North Center for Independent Living of Northeastern Minnesota has information and resources to help you plan your move, including Take the Road to Independence, a guide that covers housing options, finances, transportation, and many of the other details to consider when you move out of a facility.
